Below is the complete checklist. The core documents are required by all 8 consulates. Consulate-specific notes are highlighted in orange — these are where most mistakes happen.

Passport Photo All consulates
1 recent color photo. White background, full face, front view, professionally taken.
- Most consulates: 35mm × 45mm (1.37" × 1.77")
- Detroit: 2" × 2" (standard US passport size)
- Scanned, photocopied, or phone-printed photos are NOT accepted
- Glued to top-right corner of the application form

- Valid for at least 3 months beyond your last day in Italy
- Not issued more than 10 years ago
- Must have at least 2 blank pages for the visa sticker
- Your full birthdate (day, month, year) must be in the passport
- Photocopy: bio page + expiration date page + signature page (if separate)

Proof of US Residence in Consular Jurisdiction Varies by consulate
You must prove you live in that consulate's geographic area. Accepted documents vary:
- Miami: Driver's license OR State ID OR Recent Tax Return (only these 3)
- Los Angeles: Driver's License or State ID issued by CA, AZ, NV, or NM only — other states not accepted. If moved recently: utility bill required.
- San Francisco: Driver's license, State ID, medical bill, or utility bill
- Chicago / Philadelphia / New York / Detroit: Driver's license, State ID, State Income Tax, lease copy, or utility bills
- Boston: State driving license, State ID, lease copy, or last month utility bill

- New York / Philadelphia: past 5 years
- Boston: past 3 years
- Chicago / Detroit / San Francisco / LA: required but no specific timeframe stated
- Miami: not explicitly required
Travel Arrangements

Round-Trip Flight Reservation All consulates
Must show a complete itinerary from the US to Italy (and back). Names on reservations must exactly match your passport.
- Include all internal flights, Eurorail tickets, car rentals within Europe
- If visiting multiple Schengen countries: provide transportation between each country
- San Francisco: 13-digit e-ticket number must be visible on the ticket
- Boston: Print before coming — no screenshots or phone emails accepted
- Note: This is a reservation/confirmation, not a purchased non-refundable ticket

Proof of Lodging — All Nights All consulates
Must cover every night of your trip in all Schengen countries. Choose based on your situation:
- Hotels: Confirmation letter showing your name, hotel name/address, reservation dates, and confirmation number. Print the confirmation — no screenshots.
- Vacation rental (Airbnb/VRBO): Signed rental agreement
- Staying with family/friends in Italy: A Declaration of Hospitality (host declaration form) — must be signed and dated by host, accompanied by copy of host's Italian ID or passport (and permesso di soggiorno if host is non-EU). Boston: host must email declaration directly to visti.boston@esteri.it. Miami/SF: must be physically mailed in original.
- Organized tour: Official tour itinerary + letter from travel agent confirming group membership
Financial Documents

Proof of Financial Means — Bank Statements Varies by consulate
Must meet minimum balance requirements (per Italian Ministry of Interior Directive March 1, 2000). All accounts must be US-based banks.
- San Francisco / Chicago / Philadelphia / New York / Detroit: Last 3 months of complete bank statements (checking; savings also accepted)
- Miami: Recent bank statement or banker's letter
- Boston: Latest personal bank statement from US bank
- Los Angeles: Last 3 months bank statements AND last 3 months pay slips (both required)

Employment / Income Verification All consulates
Provide the relevant document for your situation:
- Employed: Letter on company letterhead stating your position, length of employment, annual/monthly salary, and approved vacation period with dates. Must be signed (some consulates require wet signature) and dated. New York, Chicago, Philadelphia, LA: also include last 3 pay stubs.
- Self-employed: Valid business license + certificate of incorporation + latest business tax return + last 3 months business bank statements
- Student: Official school letter confirming full-time enrollment + valid I-20 with travel endorsement (if applicable)
- Retired: Proof of pension — letter from Social Security, former employer letter, or pension check stub. Pension must be from US source — foreign pensions not accepted (NY, Philadelphia, Chicago)
- Unemployed / dependent: Sponsor's employment docs + bank statements + notarized Affidavit of Support. Dependent spouse: joint tax return or marriage certificate. Dependent children: birth certificate.
- Not employed and not sponsored: No document required (Miami, SF)
Insurance

Travel Medical Insurance Most common rejection reason
Must be a letter or certificate from the insurance company (not just your insurance card). Must explicitly state:
Your existing US health insurance may qualify — call them and request a letter confirming overseas coverage with the required benefits listed.
- Coverage for medical expenses, hospitalization, medical evacuation, and repatriation of remains
- Minimum coverage: €30,000 / $50,000
- Covers the entire duration of your trip outside the US
- Your name, policy dates, and specific benefit amounts
Your existing US health insurance may qualify — call them and request a letter confirming overseas coverage with the required benefits listed.
Fee & Return Envelope

Visa Application Fee — Money Order or Cashier's Check All consulates
Non-refundable, regardless of outcome. The fee adjusts quarterly (Jan 1, Apr 1, Jul 1, Oct 1) based on the official EUR/USD exchange rate. Check your specific consulate's website for the current amount.
- Payment: Money Order or Cashier's Check only (no cash, no personal checks)
- Made out to: "Consulate General of Italy" + your consulate city
- Detroit: payable to "Consulate of Italy in Detroit"
- LA: payable to "Consulate General of Italy – Los Angeles"

Self-Addressed Pre-Paid Return Envelope All consulates
Your passport with the visa will be mailed back to you after processing. Each applicant needs one.
- Self-addressed: both "TO" and "FROM" must be YOUR name and address
- Pre-paid: USPS Priority Mail, Express Mail, FedEx, or UPS (check your consulate)
- Chicago: recommends UPS; both TO and FROM must be applicant's name/address
- Los Angeles: Consulate's name as sender will NOT be accepted
- Strongly recommend purchasing tracking — consulates do not offer tracking
- Detroit: No in-person pickups — return envelope is mandatory
📋 If Applying for a Minor Child
In addition to all documents above, minors require:
- → Birth certificate (original + copy; non-English must be translated and legalized)
- → Parental travel authorization — signed by both parents, notarized by US notary (or Italian consulate if parent is abroad)
- → Minors under 12: parent submits on their behalf; children 12 and older must appear in person
- → New York: BOTH parents must come to the interview; divorce does not grant sole authority
- → Los Angeles: Declaration of financial responsibility (notarized) + notarized copies of both parents' IDs
- → If travelling without a parent (school trip): letter from school + chaperone assumption of responsibility (notarized)
