Fill Out Your Visa Application Correctly

Errors on the application form are a leading cause of delays and rejections. Follow this guide — or let our experts fill it out for you.

Go to the Official Form Portal
Visit e-applicationvisa.esteri.it — the official Italian Ministry of Foreign Affairs form portal. This is the only accepted source for the Schengen visa application form. Do not use third-party form generators.
Tip: Fill out the form online, then print it in high resolution (minimum 300 DPI). Some consulates — especially Los Angeles — strictly require high-quality prints and will reject blurry or low-resolution forms.

Personal Information — Match Your Passport Exactly
Enter your name exactly as it appears on your passport — same spelling, same order. This includes:
  • Full legal name (no nicknames)
  • Date of birth: Day / Month / Year format
  • Place of birth: city and country
  • Nationality as shown on passport
  • Passport number, issue date, expiration date, issuing authority
⚠️ Any mismatch between your form and your passport — even a middle name or a letter — can result in rejection.

Travel Information
  • Destination country: Italy (or whichever country you'll spend the most time in)
  • Number of entries requested: Single entry (one trip) or Multiple entry (multiple trips within validity)
  • Intended date of first entry: Your planned arrival date in the Schengen Area
  • Intended date of last exit: Your planned departure date from the Schengen Area
  • Duration of stay: Number of days
Tip: If visiting multiple Schengen countries, the country where you'll spend the most nights is your "main destination" — that country's consulate processes your visa.

Purpose of the Journey
Select the primary reason for travel:
  • Tourism — for sightseeing and leisure
  • Visiting family or friends
  • Business
  • Study / educational purposes
  • Other (specify)
Be honest and consistent — the purpose you state must match your supporting documents (hotel bookings, employment letter, etc.).

Financial Means & Accommodation
State how you will cover your costs:
  • Cash / personal funds
  • Sponsored by a family member (attach their affidavit)
  • Sponsored by employer or inviting organization
For accommodation: list your hotel name and address, or your host's address if staying with family/friends.

Do NOT Sign — Sign at the Consulate
Leave the signature field blank. You must sign the application form in front of the consular officer at your appointment. Signing beforehand may invalidate your application.
Remember: Glue your passport photo to the top-right corner of the printed form before your appointment.
✗ Wrong — Signing the form at home
✓ Right — Sign only in front of the officer
The consular officer must witness your signature. Pre-signed forms are grounds for rejection.
✗ Wrong — Name doesn't match passport
✓ Right — Exact match, including middle names
Even a missing middle name or different spelling causes issues. Copy from passport exactly.
✗ Wrong — Low-resolution printed form
✓ Right — Print at minimum 300 DPI
Los Angeles and other consulates reject blurry or poorly printed forms. Use a quality printer.
✗ Wrong — Trip dates don't match flight/hotel docs
✓ Right — All dates consistent across all documents
Your application dates, flight reservation, and hotel bookings must all show the same travel window.
✗ Wrong — Photo glued to wrong spot
✓ Right — Top-right corner of the form
The photo goes in the designated top-right box on the first page. Don't staple — glue it.
✗ Wrong — Using old or wrong form version
✓ Right — Always use e-applicationvisa.esteri.it
Only the form from the official Italian portal is accepted. Third-party versions may be outdated.
