Online vs PDF — What's the Difference?

Online form (e-applicationvisa.esteri.it): you fill in your details on the official portal, which generates a completed form with a barcode. This is the current standard and is accepted by every consulate.

PDF form: some consulates also publish a downloadable PDF (a fillable form or a completed sample for reference). Print it and fill it legibly if your consulate provides one.

How to Fill It Out — Step by Step

1. Pick your consulate's form from the table above (online or PDF).
2. Fill every field in English, matching your passport exactly (name, DOB, passport number).
3. Use matching dates across the form, flight reservation, hotel bookings, and insurance.
4. Do not sign — leave the signature field empty until your appointment.
5. Print legibly (high resolution, 300 DPI) and attach your photo where required (Miami and San Francisco: glue to top-right).
6. Bring the printed form plus all documents to your appointment.

Common Mistakes to Avoid

• Signing the form before the appointment — it will be rejected.
• Using the wrong consulate's form (each is different).
• Name or dates that don't match your passport and bookings.
• Low-resolution or screenshot prints (min 300 DPI required).
