Required Documents

Bring a complete set per person. Requirements vary by consulate.

1. Visa application form filled out but NOT signed (sign before the officer)
2. Newly taken 2×2 photo, white background, no shadows (paper copies not accepted)
3. Original passport + 1 photocopy
4. Original driver's license / State ID + 1 photocopy
5. Original US visa / Permanent Resident Card + 1 photocopy (students: F-1 + I-20)
6. Copies of most recent Schengen visas
7. Confirmed round-trip flight (complete itinerary + applicant name)
8. Proof of lodging (hotel with name/address/check-in/out; or invitation letter + host Italian ID)
9. Employment verification letter on letterhead (self-employed: license + tax return; students: enrollment; retirees: pension)
10. Three most recent bank statements from a US bank (no screenshots)
11. Medical insurance: minimum $50,000 (hospitalization, emergencies, evacuation, repatriation); cards/booklets NOT accepted
12. Visa fee: money order to 'Consulate of Italy in Detroit'
13. Prepaid self-addressed envelope with tracking (USPS/FedEx); no in-person pickups
