Required Documents

Bring a complete set per person. Requirements vary by consulate.

1. Application form filled online and printed in HIGH RESOLUTION (min 300 DPI), signed before the officer
2. 35mm × 45mm photo, white background, color (no scanned/photocopied)
3. Passport valid 3 months beyond stay, <10 years old, 2 blank pages, with full birthdate
4. Driver's License / State ID from CA, AZ, NV or NM only (else current water/gas/electric bill)
5. Proof of US residence: Alien Registration Card or long-term visa (A/E/F/G/H/I/J/L/O/R) + I-94, valid 3 months beyond departure
6. Bank statements (last 3 months) AND proof of income (last 3 pay slips; retirees: pension certificate)
7. Employment verification (letter with WET signature; self-employed: license + incorporation + tax return; students: enrollment + endorsed I-20)
8. Round-trip flight reservation (+ transport between Schengen countries)
9. Proof of lodging (print-outs only, no phone screenshots; or Declaration of private accommodation + host ID + Permesso)
10. Travel insurance: minimum €30,000 (medical, hospitalization, repatriation)
11. Fee: Money Order / Cashier's Check to 'Consulate General of Italy – Los Angeles'
12. Self-addressed pre-paid envelope (Express/FedEx/USPS); return label must NOT show consulate as sender
