Required Documents

Bring a complete set per person. Requirements vary by consulate.

1. Passport valid 3 months beyond last day of stay, 2 blank pages (+ copy of data/expiration/signature pages)
2. Proof of physical residence: driver's license, State ID, or recent tax return (address in jurisdiction)
3. Non-US citizens: Alien Registration Card or long-term US visa (NOT B1/B2), valid 3 months beyond departure (+ copy); students: I-20, exchange: I-66
4. One recent ICAO photo: 45mm × 35mm, color, glued to top-right of form
5. One application form completely filled (or e-applicationvisa.esteri.it)
6. Proof of funds: recent bank statement or banker letter (min per Interior Ministry Directive)
7. Employment verification letter (position, salary, leave dates; self-employed: license + tax return; students: enrollment; unemployed: none)
8. Round-trip ticket from jurisdiction to Europe/Italy and back (+ internal transport)
9. Lodging for all nights (hotel letter; or original Declaration of Hospitality mailed by host + host ID)
10. Worldwide travel insurance: min €30,000 (or $50,000)
11. Fee: cashier's check or money order to 'Consulate General of Italy' (exact amount)
12. Declaration for Mailing Passports (filled out)
13. Self-addressed pre-paid USPS Priority/Express envelope (one per applicant)
