  3. Valid passport + 1 copy (3 months beyond visa, <10 years, 2 blank pages)
  4. Original + 1 copy of Permanent Residence Card / US immigration document (students: F-1 + I-20 travel signature)
  5. Confirmed round-trip flight reservation (+ transport between countries)
  6. Medical insurance: min €30,000 (or $50,000) for hospitalization, emergencies, evacuation, repatriation
  7. Proof of accommodation (hotel; or host declaration + host ID + Permesso)
  8. Three most recent US bank statements (dependents: Affidavit of Support)
  9. Employment verification
  10. Visa fee: Money Order/Cashier's check to 'Embassy of Italy in Washington D.C.' or cash (exact amount)
  11. Passport collection: Mon–Thu 10:00–11:30 am (no mail-in unless arranged)
