Italy Visa Documents — Chicago Consulate

Required Documents

Bring a complete set per person. Requirements vary by consulate.

1. Visa application form, dated and signed in front of the Visa Officer
2. Recent passport photo, white background, stapled/glued to the form (no scanned/photocopied photos)
3. Valid passport + 1 copy of the photo/expiration page (valid 3 months beyond visa, <10 years old, 2 blank pages)
4. Original + 1 copy of Permanent Residence Card or US immigration document (students: F-1 + I-20 travel signature)
5. Driver's License / State ID as proof of residence (utility bill if moved; bank statements NOT accepted)
6. Copies of most recent Schengen visas + entry/exit stamps
7. Confirmed round-trip flight reservation + transport between countries
8. Proof of lodging (hotel with name/address/guests/dates; or host declaration + host ID + Permesso)
9. Employment verification (letter; self-employed: corporation papers + license + tax return; students: enrollment; pensioner: pension proof)
10. Three most recent US bank statements
11. Medical insurance: minimum $50,000 for hospitalization, emergencies, evacuation, repatriation (4 benefits itemized)
12. Visa fee: Money Order or Cashier's check to 'Consulate General of Italy in Chicago' (applicant as remitter)
13. Pre-paid self-addressed trackable return envelope (TO/FROM both = applicant; UPS recommended)
