Required Documents

Bring a complete set per person. Requirements vary by consulate.

  1. Schengen visa application form, duly filled in and signed
  2. One recent passport-style photo (professionally taken, full front, white background)
  3. Passport valid 3 months beyond planned stay, at least 2 empty visa pages (+ copy of main page)
  4. Non-US citizens: Alien Registration Card or valid USCIS visa (I-20 / IAP-66 / I-797 / Advanced Parole) + copy
  5. Proof of residence: driver's license, State ID, lease, or last month's utility bill (+ copy)
  6. Copies of all Schengen visas issued in the past 3 years
  7. Round-trip flight to Italy and out of Schengen area (printed)
  8. Proof of accommodation: hotel/Airbnb with address + applicant name; or host declaration (host emails it directly to [email protected])
  9. Employment verification (letter + latest pay stub; self-employed: business bank statements + taxes + incorporation; retired: Social Security; students: enrollment letter)
  10. Latest personal bank statement from a US bank (printed)
  11. Travel medical insurance covering the entire stay
  12. Visa fee paid by money order
  13. Self-addressed prepaid return envelope

Boston FAQ

Does Boston require two sets of documents?

No. Boston requires one set (original + copies as noted). Only Los Angeles requires two complete sets.

Can I book through a third-party service?

No. Appointments obtained through third-party services are not valid and will be automatically cancelled. You must book via PRENOT@MI yourself.

What insurance coverage do I need?

Boston does not state a fixed minimum; your policy must cover medical, hospitalization, evacuation and repatriation for the entire stay.
