  11. Compulsory round-trip flight reservation (transport between countries if multiple)
  12. Visa fee: USPS Money Order to 'Consulate General of Italy'
  13. Self-addressed pre-paid Express Mail envelope (one per household)
  14. Minors: travel authorization by both parents + parents' IDs + birth certificate (translated/legalized)
