Required Documents

Bring a complete set per person. Requirements vary by consulate.

1. Visa application form, dated and signed before the Visa Officer (names must match passport)
2. 2×2 inch (51×51mm) photo, white background (no scanned/photocopied)
3. Valid passport + 1 copy (3 months beyond visa, <10 years, 2 blank pages)
4. Permanent Residence Card / US immigration doc (students: notarized F-1 + I-20)
5. Driver's License / State ID (utility bill/bank statement if recently moved)
6. Copies of most recent Schengen visas
7. Employment verification (letter OR pay stubs; self-employed: corp papers + license + tax return; students: enrollment/transcript)
8. Financial: 3 recent bank statements, min $100 per person per day (dependents: Affidavit of Support)
9. Medical insurance: minimum €30,000 (4 benefits itemized)
10. Proof of lodging (hotel; or host declaration + host ID + Permesso)
11. Compulsory round-trip flight reservation (transport between countries if multiple)
12. Visa fee: USPS Money Order to 'Consulate General of Italy'
13. Self-addressed pre-paid Express Mail envelope (one per household)
14. Minors: travel authorization by both parents + parents' IDs + birth certificate (translated/legalized)
