Italy Visa Documents — San Francisco Consulate

  1. Passport valid 3 months beyond stay, 2 blank pages (+ copy of name/DOB/signature/expiration)
  2. Proof of residence: driver's license, State ID, medical or utility bill (photocopy)
  3. Non-US citizens: Alien Registration Card / long-term US visa (NOT B1/B2), 3 months beyond departure (+ copy); students: I-20, exchange: I-66
  4. One ICAO photo: 45mm × 35mm, color, glued top-right
  5. One application form (or e-applicationvisa.esteri.it)
  6. Three most recent bank statements (min per Interior Ministry Directive)
  7. Employment verification letter (position, salary, leave dates; self-employed: license + tax return; students: enrollment; unemployed: none)
  8. Round-trip ticket (13-digit e-ticket number must be visible)
  9. Lodging for all nights (hotel; or host mails original Declaration of Hospitality + host ID)
  10. Travel insurance: min €30,000 (or $50,000); inadequate coverage is the #1 rejection reason
  11. Fee: cashier's check / money order to 'Consulate General of Italy'
  12. Declaration for Mailing Passports (filled out)
  13. Self-addressed pre-paid USPS envelope (one per applicant)
